Iron: More Than Just Energy
We often think of iron as the key to fighting fatigue, but the reality is more complex. Iron, stored as ferritin in our bodies, is fundamental for oxygen transport, immune function, cognition, and cellular repair. When ferritin, our best marker of iron storage, drifts outside an optimal range, subtle symptoms can begin to signal deeper imbalances.
Ferritin: The Iron Storage Marker
Ferritin is a protein found inside cells, acting as a safe storage vessel for iron. Unlike serum iron or hemoglobin, ferritin gives us insight into the body's reserves. It's a reliable early warning sign—sometimes dropping or rising before other labs show a problem. This is why we pay close attention to ferritin, not just hemoglobin, when evaluating iron status in both men and women.

Low Ferritin: Root Causes and Signals
Low ferritin can stem from several root causes: chronic blood loss (heavy menstrual periods, GI bleeding), inadequate dietary intake, poor absorption (due to gut issues or medications), or increased demand (pregnancy, athletic training). Women, particularly those with periods, face a higher risk, but men are not immune—especially with digestive conditions or chronic illness.
Symptoms of low ferritin often masquerade as “just being tired”: brain fog, hair shedding, restless legs, cold intolerance, and even mood changes. You might feel winded during exercise or notice your stamina dipping. When ferritin drops, the body can’t efficiently make the hemoglobin needed for red blood cells, but symptoms can arise before anemia develops.
High Ferritin: Too Much Iron, or Something Else?
When ferritin is too high, the story gets more complicated. Elevated ferritin might indicate iron overload, but it can also reflect inflammation, liver dysfunction, infection, or metabolic syndrome. Men are more likely than women to develop true iron overload (hereditary hemochromatosis is one example), yet postmenopausal women are also at risk.
If your ferritin is high, we look closely at the context: are other iron markers (like transferrin saturation) also elevated? Is there evidence of inflammation or infection? High ferritin from excess iron can damage organs over time, but high ferritin from inflammation points to underlying issues needing attention.

Men and Women: Different Risks, Different Clues
For women of reproductive age, low ferritin is more common—menstrual blood loss and pregnancy both increase the need for iron. Even slight drops can impact energy, mood, and hair. For men and postmenopausal women, we’re more vigilant about high ferritin, especially if there’s a family history of iron overload, unexplained fatigue, or metabolic conditions.
It’s easy to miss iron imbalances, especially if symptoms are mild or overlap with other issues. We encourage our patients not to dismiss persistent fatigue, cognitive changes, or changes in physical performance. Sometimes, a simple ferritin test is the missing puzzle piece.
Beyond the Numbers: The Goldilocks Principle
Optimal ferritin isn’t just about avoiding deficiency or excess. Too little iron impairs oxygen delivery, but too much can create oxidative stress and tissue damage. Most labs set a wide reference range, but the true optimal range is narrower and varies by individual context.
For women, optimal ferritin is often above 40–50 ng/mL (some sources suggest higher for those with hair loss), while for men it’s critical to avoid creeping above 200–300 ng/mL without cause. We always interpret lab results in the context of symptoms, medical history, and repeat testing as needed.

What Can You Do?
- Monitor trends, not just snapshots. If you feel persistently unwell, ask about your ferritin—not just a generic “iron panel.”
- Diet matters, but so does absorption. Red meat, lentils, and leafy greens provide iron, but gut health, vitamin C intake, and avoidance of inhibitors (like excess coffee or antacids) are just as important.
- Don’t supplement blindly. Iron supplements can help when deficiency is confirmed, but unnecessary supplementation—especially in men or postmenopausal women—can do more harm than good.
- Address root causes. If ferritin is off, we look for chronic blood loss, gut dysfunction, inflammation, or metabolic issues. Treating the root, not just the number, is key.
